Requirements for Entrance Examination for Senior High School
- Photocopy of JHS Card with 1st to 2nd Quarter Grade duly authenticated by the principal
- General average of at least 85% of above
- Two (2) pcs passport size picture with name tag preferably blue background
- Certification that he/she is currently enrolled in Grade 10
- Non-refundable fee of Php 300.00 to be paid at the Cashier
- Duly accomplished application form
Steps for Entrance Examination for Senior High School
- Present the 1st three (3) requirements to the assigned screening committee (Photocopy of Card, 2pcs passport size picture and certification)
- Pay the entrance examination fee of PHP 300.00 at the Cashier’s Office then proceed to Senior High School building
- Fill out the application form, to be given once you qualify to take the exam
- Submit the duly accomplished form for your exam schedule and room assignment
Requirements for Incoming Grade 11 Students’ Enrollment
- Must be passer of RTSHS
- Report Card with General Average of 85% and above (Photocopy and Original)
- Certificate of Good Moral Character (Photocopy and Original)
- 2 pcs passport size pictures with white background and name tag
- Photocopy of PSA/NSO Birth Certificate (3 copies)
- 1 pc yellow expanded envelope and 1 long folder
- Hair cut for male students (No hair color for both Male and Female)

- Report Card
